–––You are a pioneer in preventive medicine and anti-aging. Could you tell us how you started on this path?
Dr. Tsukada: I decided in 1999 to dedicate my life to being a surgeon. Initially, I worked in general surgery, but in July 2009, I had an accident that left me unable to use my right hand freely, which meant I could no longer serve as the kind of surgeon who directly saves lives. While recovering from severe injuries and undergoing palliative treatment, I began to question the limitations of current medicine, which primarily relies on symptomatic treatments. For example, if a tooth hurts, the pain is treated. If blood pressure is high, medication is given to lower it. While such treatments are necessary in their own way, I started to believe that the future of medicine should prioritize prevention—treating conditions before they manifest as diseases. This includes reevaluating lifestyles, using quality supplements, engaging in appropriate exercise, and understanding one’s own body. This awareness led me to realize the importance of preventive medicine, which focuses on addressing “pre-disease” states to improve overall health and avoid illness.

Reevaluating Lifestyle: “Diet, Exercise, Rest, Environment, and Mental Health”
–––What is necessary for reevaluating lifestyle habits?
Dr. Tsukada: I believe the three most important factors in lifestyle are “diet, exercise, and rest.” To this, I would add mental health as another critical element. When we ask, “What does it mean to be healthy?” as defined by the WHO, health isn’t just about physical well-being; mental well-being is equally important. As people age, their focus on health increases, but so do risks from various perspectives. Changes in environment can also affect health. Therefore, reevaluating “diet, exercise, rest, environment, and mental health” is essential.
① For “diet,” it’s important to consume plenty of vegetables and fermented foods.
② For “exercise,” even walking is good. For example, figuring out how to walk 10,000 steps effectively is key, and building muscle, especially around the hips and back, is crucial. Japanese people, being an agricultural people, tend to slouch forward, which makes it easier for front muscles to develop but harder for back muscles to strengthen. Westerners’ well-developed back muscles and posture allow them to wear suits elegantly.
③ For “rest,” achieving quality sleep and finding effective ways to reset oneself are vital.
④ “Environment” should be considered to adapt to changing circumstances.
⑤ Lastly, “mental health” involves whether one is alone, has a partner or family, or is part of a community, as well as maintaining emotional stability.
